Get to know this plan

  • Your relationship with your primary care provider (PCP) is important. Select your favorite network provider or have us match you with a PCP. You can change your PCP at any time. See them for preventive care like your annual checkup, managing chronic conditions, and everyday concerns.
  • Know the importance of a referral from your PCP. To make sure the services are covered, your primary care provider must submit a referral to us before you can see a specialist. Without a referral, you may be responsible for the full cost of the service.
  • Stay in network and save money. Be sure to check that your provider is in-network to avoid any surprise costs. Services received from providers outside of the network will not be covered (except for emergency care) and you may be responsible for the full cost of the service.
  • It’s easy to search our network.
